Complete an Exit Assessment

In this topic, you will learn how to complete an exit assessment.


Steps:

  1. Click the Application Review menu item to begin the review process.

  2. Click the button to the right of the Form Type field to display the list of form types.

  3. Select the Exit Assessment Form list item.

  4. Click the Select button to mark Panel 2.

  5. Click the Continue > button to move to the Main Questions tab of the exit assessment.

  6. The form opens on the Main Questions tab.  The Question Summary tree is displayed on the left-hand side.

     

    Note:  As soon as you engage this screen, the system displays a clock on the right-hand side which will count down to the standard timeout for a G5 session.  The session will timeout in 20 minutes, however, you will be able to click a button to continue your session without interruption.

  7. Click the Expand Menu button to expand the General Comments menu.

  8. The questions tree expands to display the question group.

  9. Click the Expand Menu button to expand the Overall Comments question group.

  11. Note:  You can expand all the question groups which will allow for easy navigation between questions.

  12. Click the Vertical scrollbar to scroll down and view the action buttons.

  13. NoteIf you wish to submit your assessment anonymously, you may check the Submit Anonymously box.  You may also save your work as often as you wish by clicking the Save button.

  14. Click the Overall Comments menu item to display the first question.

  15. The first question displays.  For each question, there is an Answer section and an Answer Complete box.

     

    Questions will either be free-form where you can write a narrative or have a list from which you will select the answer.  Note:  The answer box will be limited to 4000 characters.

  17. Note:  You may spell check your work at any point by clicking the button.

     

    Click the Answer Complete? option to indicate that you are finished with the question.

  18. Click the Accept Changes button.

  19. Note:  After you click the Accept Changes button, the first question is updated in the Question Summary tree with a green check mark.

  20. Click the Save button to save your work.

     

    Note:  You may save your work as often as you like and it is a good practice to save after each completed question, although it is not required.

  21. You may repeat the process for each of the questions.  This is a sample of a completed Main Questions tab.

  22. Click the Continue > button to continue to the Summary tab.

  24. Note:  The saves you have made at this point have changed the status of the Exit Assessment to Draft.  For the system to consider the assessment as submitted, you must actually click the Submit button.

     

    Click the Submit button to submit your work.

  25. The system displays a message indicating that the assessment form has been successfully submitted.
